What the quote form collects.
This note describes the route inquiry on this website. It is not a substitute for a counsel-reviewed privacy policy, and it does not describe a patient record system.
Information the form asks for
Business name, your name, a work email, an optional work phone, the service town, the planned frequency, and the cargo category. Do not enter patient names, dates of birth, specimen identifiers, diagnoses, or other clinical details. The form rejects wording that looks like that information.
When an inquiry is delivered
The server sends the inquiry by email only when a recipient and a mail provider are configured, and only after that provider accepts the message. Until then, the page says the inquiry was not delivered. Inquiries are not written to a database. Server logs record the outcome, such as accepted or not configured, and do not record the names, email address, or phone number from the form.
What this site does not keep
There is no customer account, no patient chart, and no shipment file on this website. A later tracking portal, if one is used, would be a separate courier platform with its own terms.
Spam controls
The form includes a field people should leave empty, a short waiting period, and a limit on repeated submissions from the same network. Those controls do not store the contents of an inquiry.
